          目的:分析心衰合并心房颤动患者血清脑钠肽(Brain Natriuretic Peptide, BNP)水平变化及其临床意义。方法:选取我科2009年1月到2013年6月住院治疗的120例房颤或者房颤并发其他并发症患者,其中48例为心房颤动并发重度心衰(A组);持续性或持久性房颤伴明显心衰前期症状患者44例(B组);阵发性房颤患者38例(C组);同时选取对照组健康正常人群50例作为对照组。测定四组的C-反应蛋白(CRP)、纤维蛋白原(Fib)、BNP和氨基末端B型钠尿肽前体(NT-pro BNP)水平,同时对四组进行心脏彩超检查,测定其左心房内径(LAD)并进行对比。分析CRP、Fib、BNP、NT-pro BNP、左心房内径五个属性间的相关关系。结果:各组间CRP、Fib、BNP、NT-pro BNP、左心房内径对比均为A组>B组>C组>对照组,P<0.05。CRP和NT-pro BNP、左心房内径之间存在正相关性(r=0.875,P=0.037和r=0.384,P=0.028)。Fib和BNP、NT-pro BNP、左心房内径之间存在正相关性(r=0.226,P=0.012;r=0.383,P=0.043;r=0.428,P=0.046)。BNP和Fib、NT-pro BNP、左心房内径之间存在正相关性(r=0.351,P=0.034;r=0.452,P=0.012;r=0.475,P=0.033)。NT-pro BNP和CRP、Fib、BNP、左心房内径之间存在正相关性(r=0.689,P=0.009;r=0.178,P=0.016;r=0.834,P=0.008;r=0.735,P=0.024)。左心房内径和CRP、Fib、BNP、NT-pro BNP之间存在正相关性(r=0.714,P=0.007;r=0.215,P=0.027;r=0.632,P=0.044;r=0.679,P=0.035)。结论:CRP、Fib、BNP、NT-pro BNP、左心房内径可作为心力衰竭合并心房颤动患者的病情发展及预测指标。

          To analyze the serum brain natriuretic peptide (BNP) and its clinical significance in patients with heart failure and atrial fibrillation. Methods: 120 cases of atrial fibrillation or other complications in our department from January 2009 to June 2013 hospitalization, including 48 cases of severe heart failure complicated by atrial fibrillation (A group); persistent or persistent atrial fibrillation in patients with early symptoms of overt heart failure 44 cases (group B); 38 patients with paroxysmal atrial fibrillation (group C); while the control group of healthy normal people selected 50 cases as the control group. There was a positive correlation (r = 0.689, P = 0.009 between BNP and CRP, Fib, BNP, left atrial diameter NT-pro; r = 0.178, P = 0.016; r = 0.834, P = 0.008; r = 0.735, P = 0.024). There was a positive correlation (r = 0.714, P = 0.007 between the left atrium and CRP, Fib, BNP, NT-pro BNP; r = 0.215, P = 0.027; r = 0.632, P = 0.044; r = 0.679, P = 0.035). Conclusion: CRP, Fib, BNP, NT-pro BNP, left atrial diameter can be used as a predictor of disease progression in patients with heart failure and atrial fibrillation.
