摘要: 目的 分析冠状动脉扩张症(CAE)患者血浆胰岛素水平及其与基质金属蛋白酶-9(MMP-9)/基质金属蛋白酶组织抑制因子1(TIMP1)比例失衡的关系。方法 选择本院行冠脉造影患者80例,根据造影结果分为冠脉扩张组与冠脉正常组,均为40例。再根据冠脉斑块情况分亚组:A1组,18例,单纯冠脉扩张;A2组,22例,冠脉扩张伴少量斑块;B1组,25例,冠脉正常不伴狭窄或斑块形成;B2组,15例,冠脉无明显狭窄但有散在斑块。检测各组血浆胰岛素、MMP-9和TIMP1水平,计算MMP9/TIMP1比例。分析血浆胰岛素水平与MMP-9、TIMP1水平的相关性,应用多元Logistic回归分析评估CAE发生的危险因素。结果 与冠脉正常组相比,冠脉扩张组患者的血浆胰岛素、MMP-9水平及MMP-9/TIMP1比值较高,而TIMP1水平较低(均P<0.05)。亚组分析显示,血浆胰岛素、MMP-9水平及MMP-9/TIMP1比值顺序为A2组>A1组>B2组>B1组(P均<0.05),而TIMP1水平为B2组> B1组> A2组> A1组(P <0.05)。相关性分析结果显示血浆胰岛素水平与MMP-9呈正相关(r=0.572,P<0.01),与TIMP1呈负相关(r=-0.568, P<0.01);MMP-9与TIMP1呈负相关(r=-0.468,P<0.01)。血浆胰岛素及MMP-9/TIMP1比值是CAE发生的危险因素(P <0.05)。结论 CAE患者血浆胰岛素水平较高,可能通过调节血浆MMP-9和TIMP1水平比值参与CAE发生。

Abstract Objective To analyze the plasma level of insulin, matrix metalloproteinase -9 (MMP-9) and tissue inhibitor of metalloproteinases 1(TIMP1) in patients with coronary artery ectasia (CAE), and to explore the correlation of plasma insulin, MMP-9 and TIMP1. Methods The 80 cases undergoing coronary angiography (CAG) in our hospital were collected and divided into two groups according to the result of CAG, which were the CAE group and the normal group. Each group had 40 cases. And each group was divided into two sub-groups, which were A1 sub-group (18 cases, simple CAE), A2 sub-group (22 cases, CAE combined little plaque), B1 sub-group (25 cases, coronary artery completely normal), B2 sub-group (15 cases, simple little plaque). The plasma levels of insulin, MMP-9 and TIMP1 were detected and compared among all groups and sub-groups. The correlations of plasma insulin, MMP-9 and TIMP1 were analyzed. The risk factors of CAE were analyzed by multiple Logistic regression analysis. Results Compared to normal group, the plasma levels of insulin, MMP-9 and the ratio of MMP-9/TIMP1 in CAE group were higher, but the plasma levels of TIMP1 in CAE group were lower (P<0.05). The sequences of insulin, MMP-9 and the ratio of MMP-9/TIMP1 in four sub-groups were A2 sub-group>A1 sub-group>B2 sub-group>B1 sub-group (P<0.05), but the sequences of TIMP1 were B2sub-group> B1 sub-group> A2 sub-group> A1 sub-group (P<0.05). The positive relationship of insulin and MMP-9 (r=0.572, P<0.01), and the negative relationship of insulin and TIMP1 (r=-0.568, P<0.01), and the negative relationship of MMP-9 and TIMP1 (r=-0.468,P<0.01) were confirmed. The plasma levels of insulin and the ratio of MMP-9 /TIMP1 were risk factors to CAE by multiple Logistic regression analysis. Conclusions The plasma level of insulin was high in CAE patients, and took part in CAE by modulation of the ratio of MMP-9 /TIMP1.
