          目的 观察肺源性心脏病(肺心病)患者缓解期、急性加重期的血清精氨酸加压素、和肽素的变化特征,探讨两者与动脉血气分析的相关性,以期评估血清精氨酸加压素、和肽素对肺心病患者病情进展和预后疗效评估的临床意义。方法 98例研究对象分为三组,包括健康对照组30例、肺心病缓解期36例、肺心病急性加重期32例,所有研究对象入院后采用酶联免疫吸附法测定精氨酸加压素、和肽素浓度平。 结果 与健康对照组和肺心病缓解期患者对比,肺心病急性加重期患者的血清精氨酸加压素、和肽素浓度显著升高;且急性加重期的精氨酸加压素、和肽素与动脉血PH值、氧分压(PO2)呈负相关,而与动脉血二氧化碳分压(PCO2)呈正相关(P<0.05)。结论 肺心病急性发作患者的血清精氨酸加压素、和肽素的浓度显著增高。由此可见,联合检测血清精氨酸加压素、和肽素对肺心病患者病情发展评估和疗效评价具有重要的临床参考价值。

          Objective To investigate the changes of serum arginine vasopressin and copeptin and their relationships with arterial blood gas analysis in patients with pulmonary heart disease, and then to evaluate the value of serum arginine vasopressin and copeptin in the development of pulmonary heart disease. Methods Ninety-eight cases were divided into three groups, including thirty cases of healthy control group, thirty-six cases of pulmonary heart disease remission, and thirty-two cases of acute exacerbation of pulmonary heart disease. The serum levels of arginine vasopressin and copeptin of all the objects were measured by ELISA method. Result Compared with the healthy control group and pulmonary heart disease remission stage, the serum levels of arginine vasopressin and copeptin were significantly increase in the acute exacerbation stage. The arginine vasopressin, and peptide were negatively correlated with PO2 and PH in the acute exacerbation stage, but positive correlated with PCO2 (P<0.05). Conclusion Changes of serum arginine vasopressin and copeptin were closely related to the acute attack of pulmonary heart disease. Thus, combined detection of serum arginine vasopressin and copeptin in patients with pulmonary heart disease has important clinical reference and value for development evaluation and efficacy evaluation.
