| 目的：探讨经气道咳痰机对气管切开后拔管困难患者改善痰液引流及控制医院感染等方面的作用。方法：以我院ICU近三年以来收治的97例气管切开后拔管困难患者为研究对象，采用回顾性队列研究方法，分析经气道咳痰机治疗组患者与空白对照组患者在呼吸力学参数、机械通气时间、医院感染发生率等方面的差异。结果：应用经气道咳痰机患者的呼吸力学参数明显改善、机械通气时间缩短、医院感染发生率下降（P<0.05）。结论：经气道咳痰机能够显著改善气管切开后拔管困难患者呼吸功能状况、缩短机械通气时间，有效预防和控制医院感染。 |

| Objective: To investigate the effect of Cough Assist applied to extubation failure patients with tracheotomy, in terms of improving sputum drainage, hospital infection control，and so on. Method: The study included 97 cases of extubation failure patients with tracheotomy in our hospital last three years. The differences of respiratory mechanics parameters, mechanical ventilation time, the incidence rate of hospital infection between treatment group and control group were compared and analyzed by retrospective cohort study. Results: The patients who received Cough Assist treatment can get better respiratory mechanics parameters, shorter mechanical ventilation time, and lower incidence rate of hospital infection(P<0.05).Conclusion: The respiratory function status, mechanical ventilation time, and incidence rate of hospital infection in extubation failure patients with tracheotomy can be improved by Cough Assist treatment. |
