          【】 目的 探讨浅快呼吸指数(RSBI)、中心静脉血氧饱和度(SCVO2)及中心静脉血氧饱和度变化率(ΔSCVO2)与机械通气患者拔管成功率的预测价值的差异。方法 采用前瞻性研究方法,选取ICU的机械通气患者70例次,按拔管结果分成2组,比较2组SCVO2、ΔSCVO2及RSBI。结果 2组患者性别、年龄差异无统计学意义(P>0.05)。2组SBT30min SCVO2、ΔSCVO2及RSBI差异有统计学意义(P<0.05)。SBT30min SCVO2与拔管成功率正相关(r=0.283,P=0.025),ΔSCVO2与拔管成功率负相关(r=-0.425,P=0.001),RSBI与拔管成功率负相关(r=-0.282,P=0.025)。SBT30min SCVO2、ΔSCVO2和RSBI的ROC下面积分别为0.697、0.190、0.349。结论 SBT30min SCVO2、ΔSCVO2和RSBI对机械通气患者拔管成功率具有一定的预测价值,SBT30min SCVO2优于RSBI 、ΔSCVO2

          【Abtract】 Objective: Investigate differences of the forecast value of rapid-shallow-breathing index(RSBI) , entral venous blood oxygen saturation(SCVO2) and the change rate of entral venous blood oxygen saturation (ΔSCVO2)and the success rate of extubation in patients with mechanical ventilation.Methods: A perspective study was conducted. 70 patients with mechanical ventilation in ICU were divided into two groups according with extubation result.To compare the difference of SCVO2、ΔSCVO2 and RSBI. Results: No significant differences were found in the items such as age and sex among both groups(P>0.05). There were differences on SCVO2 of SBT of 30min, ΔSCVO2 and RSBI between both groups(P<0.05). There were correlation between SCVO2 of SBT of 30min and the success rate of extubation ((r=0.283,P=0.025). ΔSCVO2 correlated with the success rate of extubation negatively(r=-0.425,P=0.001). So was between RSBI and the success rate of extubation(r=-0.282,P=0.025).To compare the area of SCVO2 of SBT of 30min, ΔSCVO2 and RSBI, their areas under the ROC curve were0.697,0.190 and 0.349.Conclusion: It was certain forecast value to determine the success rate of extubation in patients with mechanical ventilation by SCVO2 of SBT of 30min, ΔSCVO2 and RSBI. It was excellentlier judged for the success rate of extubation that SCVO2 of SBT of 30min than RSBI and ΔSCVO2 .
