          目的:研究支气管肺泡灌洗联合无创正压通气对矽肺患者肺功能、血气分析及6min步行试验的影响。方法:选择自2010年2月至2014年2月于本院呼吸内科就诊的矽肺患者76例,随机分为研究组与对照组各38例,两组患者均接受平喘化痰以及无创正压通气治疗,研究组同时行支气管肺泡灌洗治疗方案。比较两组患者治疗前后肺功能(FEV1、FVC、FEV1占正常预计值%)、血气分析(PaO2、PaCO2)以及6min步行试验的差异。结果:比较两组治疗前后结果发现肺功能指标FEV1治疗后均提升且研究组(P<0.001)较对照组(P=0.011)提升更明显;FVC结果也相似;FEV1占正常预计值%均显著提升(P<0.001); 血气分析PaO2值研究组(P<0.001)比对照组(P=0.029)显著升高;PaCO2值均降低(P<0.001);6min步行距离研究组(P<0.001)较对照组(P=0.011)显著增加。结论:对矽肺患者行支气管肺泡灌洗联合无创正压通气治疗能改善矽肺患者的肺功能、血气指标及患者的运动耐量。
    英文摘要:
          Objective: To study the effect of bronchoalveolar lavage combined with noninvasive positive pressure ventilation for pulmonary function test , blood gas analysis and 6-minutes walk test in patients with Pneumoconiosis. Methods: 76 Pneumoconiosis patients collected in our hospital from February 2010 to February 2014 , divided into two groups randomly. Both patients received sputum-resolving and asthma-relieving drugs and noninvasive positive pressure ventilation, besides, the study group received bronchoalveolar lavage. Comparing the two groups pulmonary function test(FEV1, FVC, FEV1 %), arterial blood gas(PaO2,PaCO2) and 6-minutes walk test. Results: Comparing the two groups, shows that for FEV1, the study group(P<0.001)significantly improve than the control group(P=0.011); for FVC, has the same results; both groups improve the FEV1 value expected significantly(P<0.001); for the value of PaO2: the study group(P<0.001)has obviously improvement than the control group(P=0.029); both groups decrease the value of PaCO2 (P<0.001); for 6-minutes walk test:the study group (P<0.001)increase the distance significant than the control group(P=0.011). Conclusion: Bronchoalveolar lavage combined with noninvasive positive pressure ventilation can improve pulmonary function , blood gas analysis and 6-minutes walk tolerance in patients with silicosis.
