          【】目的 研究支气管哮喘患者呼出气冷凝液(EBC)中白三烯B4(LTB4)和肿瘤坏死因子(TNF-α)水平及其临床意义。方法 收集42例哮喘患者急性发作期和缓解期以及40例正常对照组的EBC,检测其中LTB4 和TNF-α水平,同时检测患者的第一秒呼气容量(FEV1)、最大呼气流速(PEF)、动脉血二氧化碳分压(PaCO2)、动脉血氧分压(PaO2)、血IgE及血白细胞总数。结果 ①哮喘患者急性发作期LTB4为36.71±15.23ng/L,缓解期为26.65±12.71ng/L,正常对照组为13.82±7.80ng/L,两两相较,差异有显著性,P均<0.05;②哮喘患者急性发作期TNF-α为16.07±7.60ng/L,缓解期为9.72±4.36ng/L,正常对照组为4.88±3.31ng/L,两两相较,差异有显著性,P均<0.05;③哮喘患者急性发作期EBC中LTB4 和TNF-α水平与血清IgE水平呈正相关,LTB4水平与PEF、FEV1占预计值的百分比呈负相关。结论EBC中LTB4 和TNF-α水平可作为评价哮喘急性发作、判断病情严重程度和观察疗效的重要指标。

          【】Objective To Study the changes and clinic significances of leukotrieneB4(LTB4) and tumor necrosis factor-α(TNF-α)in exhaled breath condensate(EBC) of asthma.Methods EBC of 42 cases in acute episode asthma,42 cases of period of remission and 40 normal persons were collected. The concentrations of LTB4 and TNF-α in EBC were assayed. Forced expiratory volume in one second (FEV1) and peak expiratory flow(PEF) of patients were observed at the same time, PaCO2, PaO2 ,Serum IgE and leukocyte were also assayed. Results ①The concentration of EBC-LTB4 in patients with acute asthma was 36.71±15.23ng/L, and that in remission of asthma was 26.65±12.71ng/L and that in normal controls was 13.82±7.80ng/L,P<0.05. ②The concentration of EBC-TNF-α in patients with acute asthma was 16.07±7.60ng/L, and that in remission of asthma was 9.72±4.36ng/L and that in normal controls was 4.88±3.31ng/L,P<0.05.③The LTB4 and TNF-α levels of EBC were positively correlated with serum IgE in patients with asthma. The concentration of LTB4 in EBC had negative relationship with the FEV1 and PEF.Conclusions The concentrations of LTB4 and TNF-α in EBC can be used to evaluate the acute attack of asthma, judge the severity of the disease and observe the therapeutic effect.
