          目的:探讨老年慢性心力衰竭患者(CHF)体内Gal-3、BNP变化意义及对CHF的辅助诊断价值。方法:入选2013年1月-2015年8月收治的90例老年CHF患者为研究组,按照性别、年龄配伍原则,选择同期心功能正常的90例老年人作为对照组,回顾性分析两组体内Gal-3、BNP变化,并对研究组入院Gal-3与BNP水平进行相关性分析并心功能分层分析,比较二者对CHF的诊断效能。结果:观察组治疗前BNP(878.5±87.3 vs. 72.8±15.2) pg/ml、Gal-3(30.3±5.9 vs. 6.2±1.5) ng/ml数值显著高于对照组(t=86.257 p<0.001;t=37.577 p<0.001),治疗14d后观察组BNP、Gal-3水平显著下降,但仍明显高于对照组水平(204.3±42.9 vs. 72.8±15.2) pg/ml 、(9.3±2.1 vs. 6.2±1.5) ng/m (t=27.410 p<0.001; t=11.763 p<0.001)。治疗前不同心功能分级组患者BNP、Gal-3水平差异有统计学意义(F=4.961 p=0.009;F=3.994 p=0.022),伴随NYHA心功能分级提高,BNP、Gal-3水平逐渐提高,BNP、Gal-3呈现显著正相关关系(r=0.884 ;t=14.567 p<0.001),二者对CHF诊断效能分别为0.832(95%CI. 0.761-0.903)、0.835(95% CI. 0.767-0.903)。结论:老年CHF患者体内BNP与Gal-3均显著升高,二者与心功能分级呈同向变化,对CHF均具有较高的诊断效能。

          Objective To investigate the variance and its significance of Gal-3 and BNP in elderly patients with CHF,and to explore the value in diagnosis of CHF. Methods 90 elderly CHF patients of January 2013 to August 2015 were selected as study group. 90 elderly patients with normal cardiac function were selected as control group according to gender and age compatibility principle, The variations of Gal-3 and BNP of the two groups were retrospectively noted and analyzed. The correlation between Gal-3 and BNP levels in the study group was analyzed, also stratified analysis was conducted based on cardiac function, besides receiver operating curve(ROC) was applied to evaluate the diagnostic efficacy of Gal-3 and BNP. Results The CHF group before treatment whose BNP (878.5 ± 72.8 vs. 30.3 ± 15.2) pg/ml, Gal-3 (5.9 ± 6.2 vs. 87.3 ± 1.5) ng/ml were significantly higher than the control group (t=37.577 p< 0.001; t=86.257 p< 0.001), treatment for 14d,BNP and Gal-3 levels significantly decreased, but still significantly higher than the control group level (204.3 ± 72.8 vs. 2.1 ± 15.2) pg/ml, (9.3 ± 6.2 vs. 42.9 ± 1.5) ng/m (t=11.763 p< 0.001; t=27.410 p< 0.001). Before treatment, there were significant differences in BNP and Gal-3 levels of patients with different cardiac function classification (F=4.961 p=0.009; F=3.994 p=0.022), with NYHA cardiac function classification increased, BNP and Gal-3 levels gradually increased, BNP and Gal-3 value showed significant positive correlation(r=0.884 ;t=14.567 p<0.001). The diagnostic efficacies of BNP and Gal-3 for CHF were 0.832 (95%CI.0.761-0.903) and 0.835 (95%CI. 0.767-0.903) respectively. Conclusion BNP and Gal-3 were significantly increased in elderly patients with CHF, and the two were in the same direction with the variation of cardiac function classification, which both had a high diagnostic efficiency for CHF.
