| 目的 比较连续性血液净化与冰毯治疗重度中暑患者的疗效。 方法 30例重度中暑患者随机分为两组,观察组和对照组各15例。观察组患者采用连续性血液净化降温,对照组采用冰毯降温,观察两组1、2、3h直肠温度下降情况及0、3、6h白细胞、C反应蛋白、降钙素原、NF-κB蛋白及热休克蛋白70(heat shock protein 70, Hsp70)表达。结果 观察组不同时间直肠温度、白细胞、C反应蛋白、降钙素原、白细胞NF-κB蛋白下降幅度大于对照组,观察组HSP70蛋白表达高于对照组。结论：连续性血液净化用于重度中暑降温幅度效果优于冰毯降温法,其机制可能为降低NF-κB蛋白,升高HSP70蛋白表达。 |

| Objective: To compare the therapy effect of combinations of continuous blood purification and ice blanket in rescue patients with severe heat stroke. Methods: 30 cases of severe heat stroke were randomly divided into two groups. Rectal temperature at 1,2,3 h was recorded in observation group with 15 cases by continuous blood purification and the control group with 15 cases by ice blanket. WBC, CRP, PCT, NF-κB protein and Heat shock protein 70 (HSP70) expression of leukocyte was measured in the two groups. Results: Rectal temperature, WBC, CRP, PCT and NF-κB protein expressing of leukocyte declined more than the control group at different times, HSP70 protein expression raised higher than the control group. Conclusion: The continuous blood purification for severe heat stroke in cooling rate is better than ice blanket machine. The mechanism may be reduce NF-κB protein expression, increased the expression of HSP70.#$NLKeywords blood purification; ice blanket; severe heat stroke |
