          目的:分析血肌红蛋白浓度(MYO)值作为经典型热射病患者预后早期判别指标的意义。方法:回顾分析2015年7月-2015年8月乌鲁木齐地区3所医院的84例热射病(heat stroke,HS)即重症中暑病例,纳入病例均为非劳力性热射病(classical heat stroke CHS)。以预后为标准, 将84例患者分为两组, 死亡组(n=52)及存活组(n=32), 比较两组病例入院后0h、12h、24h、36h、48h、60h、72h的肌红蛋白浓度数值, 并描绘48h肌红蛋白浓度值的ROC曲线, 分析肌红蛋白浓度值在发病早期判别预后的意义。结果:在入院后各时间点死亡组肌红蛋白浓度值持续升高, 存活组肌红蛋白浓度值略升高后保持稳定并逐渐下降, 两组肌红蛋白浓度值在48h、60h、72h有显著差异(P<0.05), 48h肌红蛋白浓度值的ROC曲线AUC(area under the Roc curve)为0.882。结论:死亡组与存活组肌红蛋白浓度值的变化趋势显著不同, 肌红蛋白浓度值可以在发病早期(48 h)判别预后。

          Objective: To analyze the accuracy of MYO value as a early prognostic indicator in classical heat stroke patients. Methods : We retrospectively analyzed 84 cases of CHS in 3 hospitals of Urumchi from July 2015 to Aug 2015.Interms of prognosis, the 84 patients were divided into two groups: D group (those who died, n=52)and S group (those who survived and Were discharged, n=32).We compared MYO value in two groups at 0h、12h、24h、36h、48h、60h、72hafter admission and Plotted the receiver – operating characteristic plots(ROC)curve at 48 hour to analyze the accuracy of MYO value as a prognostic indicator in the initial stage of CHS. Results: The MYO value continued to increase in D group at all time intervals after admission, The MYO value were stable and generally decreased after a mild increase, the values between the two groups had significant deviation at 48h、60 h、after admission(P<0.05). MYO ROC had an AUC of 0.882 at 48h. Conclusion: The trends of MYO in Dead patients and survival patients are significantly different, the former is significantly higher than that of the latter. MYO can be a useful indicator of the prognosis of CHS in the initial stage of CHS (48h).
