          目的 检测血清生长分化因子-15(GDF-15)在心脏骤停患者心肺复苏成功(CPR)后不同时间的水平变化,探讨血清GDF-15水平在CPR后不同时间的变化与近期临床预后的关系。 方法 分别检测102例CPR患者CPR后即刻、12h、24-48h的血清GDF-15水平,根据血清GDF-15升高时间分为3组,A组:CPR后即刻、12h、24-48h血清GDF-15水平一直小于1200ng/L, B组:CPR后12h 和24-48h GDF-15水平持续升高,且一直大于1200ng/L;C组: CPR后12h 和24-48h GDF-15水平持续升高,但24-48h较12h时降低。随访6个月后三组患者的死亡情况。 结果  随访6个月,B组患者的死亡情况明显高于A组与C组; C组患者死亡情况稍高于A组。 结论 CPR过程中,GDF-15可在心室早期激活,并且呈动态变化,这对机体是一种保护性反应。但是,CPR后,如果患者血清中GDF-15持续升高,提示患者出现了心功能不全,患者预后会很差。临床上为了改善CPR患者近期预后,治疗上应积极改善心功能不全,提高心排出量,改善重要脏器的血液灌注。

          Objective To explore the relationship between the level of growth defferentiation factor-15 in peripheral blood and the Recent clinical prognosis of cardiopulmonary resuscitation(CPR) . Methods A total of 102 victims with sudden cardiac arrest who received CPR were divided into three groups.The victims who the GDF-15 levels has been less than 1200ng/L into Group A.those who the GDF-15 levels at the time of 24-48h after CPR are higher than the time of 0h and 12h after CPR into Group B,and those who the GDF-15 levels at the time of 24-48h after CPR are lower than the time of 0h and 12h after CPR into Group C.Follow-up 102 cases with ACS 6 months of mortality incidence. Results 6 months of mortality incidence of Group B are obviously higher than that of group A and group C; 6 months of mortality incidence of Group C are slightly higher than that of group A . Conclusions The expression of GDF-15 activation at the initial and developing stage may indicate heart failure in those with CPR.At the early stage of CPR,GDF-15 has protective effects on the human body,but the massive expression of GDF-15 activation is associated with serious heart failure,which induced a high mortality of victims with CPR.
