          【】目的 探讨氧合指数等相关指标及血管外肺水指数(EVLWI)对急性呼吸窘迫综合征患者(ARDS)预后判断价值。方法 回顾性分析我院2014年3月至2015年9月入住重症监护室(ICU)的52例ARDS患者的临床资料,根据患者预后分为存活组与死亡组,分析两组患者在入住ICU病房后心脏指数(CI)、平均动脉压(MAP)、中心静脉压(CVPP)、血管外肺水指数(EVLWI)及氧合指数的变化。结果 与存活组对比,死亡组患者在入住ICU病房时的APACHEⅡ评分显著升高(P<0.05);存活组氧合指数随时间呈上升的趋势,EVLWI随时间呈下降趋势(P<0.05),死亡组氧合指数随时间呈下降的趋势,EVLWI及CI随时间呈上升的趋势(P<0.05),与死亡组第2d、3d对比,存活组第2d、3d的CI、EVLWI显著降低,氧合指数显著升高(P<0.05); 经Person相关性分析显示,EVLWI与氧合指数存在显著负相关关系,与CVP无相关性。结论 动态监测EVLWI及氧合指数对判断ARDS患者的病情及预后具有重要临床价值。

          Objective The prognostic value of oxygenation index and pulmonary water index (EVLWI) in patients with acute respiratory distress syndrome (ARDS) was discussed.Methods A retrospective analysis of our hospital from March 2014 to September 2015 admitted to the intensive care unit (ICU) patients with ARDS clinical data of 52 cases, according to the prognosis of patients divided into survival group and death group analyzed two groups of patients after admission to the ICU ward cardiac index (CI), mean arterial pressure (MAP), central venous pressure (CVPP), changes in extravascular water index (EVLWI) and oxygenation index.Results And survival comparison group and death group patients in the ICU of APACHE II score increased significantly (P < 0.05); survival group oxygen index showed a rising trend over time, EVLWI with time was decreased (P < 0.05), the death group oxygen index showed a downward trend with time, EVLWI and CI with time showed a rising trend (P < 0.05), and death group, 2D, 3D contrast, survival group, 2D, 3D Ci, EVLWI decreased significantly, oxygenation index was significantly increased (P < 0.05); after Pearson correlation analysis showed that EVLWI There was a significant negative correlation with oxygenation index, and no correlation with CVP.Conclusion Dynamic monitoring of EVLWI and oxygenation index has important clinical value in judging the condition and prognosis of ARDS patients.Keywords:Oxygenation index;External lung water index;Acute respiratory distress syndrome;Prognosis
