| 摘要 近年来，新的治疗技术如嵌合抗原受体T（CAR-T）细胞治疗及新型靶向药物的积极推进和应用，高强度化疗的临床使用，单倍体移植技术的全国推广，极大提高了血液系统恶性肿瘤患者的整体生存，但随着中老年患者比例逐渐增多，血液病危重症患者比例逐渐增加。血液病危重症患者可以早期、及时转入血液专科重症病房(HCU)，进行集中诊治和护理，加强心电监测和生命支持等，有利于改善患者的预后，提高诊治水平，降低医患纠纷风险。基于此，《中华血液学杂志》和《临床血液病杂志》均于2025年第1期发表了《血液专科重症单元危重症患者诊疗和管理中国专家共识（2025年版）》（以下简称共识）。该专家共识涵盖了HCU的配置条件、危重症患者的诊疗和管理方法，重点介绍了脏器支持治疗和管理策略，强调了个体化治疗和多学科协作的重要性。本文旨在对该专家共识进行深入解析，为我国血液内科HCU的建设和危急重症患者的管理提供建议和方向。 |

| Abstract In recent years, the active promotion and application of new treatment technologies such as chimeric antigen receptor-T (CAR-T) and novel targeted drugs, the clinical use of high-intensity chemotherapy, the nationwide promotion of haploidentical transplantation technology, have greatly improved the overall survival of patients with hematologic malignancies. However, with the increasing proportion of middle-aged and elderly patients, the proportion of critically ill hematologic patients is gradually increasing. Critical patients with hematological diseases can be transferred to the Hematological Intensive Care Unit (HCU) early and promptly for centralized diagnosis, treatment, and nursing care, as well as strengthened electrocardiogram monitoring and life support, which is beneficial for improving patients' prognosis, enhancing the level of diagnosis and treatment, and reducing the risk of doctor-patient disputes. Based on this, the "China Expert Consensus on Diagnosis and Management of Critically Ill Patients in Hematological Specialist Intensive Care Units (2025 edition)" was published in the first issue of Chinese Journal of Hematology and Clinical Hematologic Journal in 2025 (hereinafter referred to as the Consensus). This expert consensus covers the configuration conditions of HCU, as well as the diagnosis, treatment, and management methods of critically ill patients, focusing on organ support therapy and management strategies, emphasizing the importance of personalized treatment and multidisciplinary collaboration. This article aims to provide a deep analysis of the expert consensus, and provide recommendations and directions for the construction of HCU in hematology and the management of critically ill patients in China. |
