| 摘要 我国心力衰竭发病率、死亡率居高不下,成人心力衰竭患病率约0.9%,心源性休克(CS)短期死亡率达40%~50%。机械循环辅助(MCS)虽缺乏降低死亡率的随机对照试验证据,但短期MCS在急性心力衰竭重症患者中使用率显著增加。 本文综述主动脉内球囊反搏(IABP)、Impella、Tandem Heart、静脉-动脉体外膜肺氧合(VA-ECMO)及右心室辅助装置(RVAD)等短期MCS的进展,以及各装置在支持力度、安全性、适用场景上各具特点。个体化选择、严控适应证及加强并发症管理是改善预后的关键,亟需高质量研究及标准化指南优化治疗策略。 |

| Abstract The incidence and mortality of heart failure in China remain high, with a prevalence of 0.9% among adults, and the short-term mortality rate of patients with cardiogenic shock (CS) reaching 40%-50%. Although mechanical circulatory support (MCS) lacks evidence from randomized controlled trials to prove its efficacy in reducing mortality, the utilization rate of short-term MCS has increased significantly in critically ill patients with acute heart failure. This article reviews the progress of short-term MCS devices, including intra-aortic balloon pump (IABP), Impella, Tandem Heart, veno-arterial extracorporeal membrane oxygenation (VA-ECMO), and right ventricular assist devices (RVAD), each with distinct characteristics in support intensity, safety, and applicable scenarios. Individualized selection, strict control of indications, and enhanced management of complications are key to improve prognosis. High-quality research and standardized guidelines are urgently needed to optimize the treatment strategy for patients with acute heart failure. |
