| 目的 观察连续性血液净化联合机械通气治疗难治性心衰合并呼吸衰竭的临床疗效。方法 选择我院ICU收治42例接受连续性血液净化联合机械通气治疗的难治性心衰合并呼吸衰竭的患者。在治疗前后监测血流动力学及组织灌注指标——心率（HR）、平均动脉压（MAP）、中心静脉压（CVP）、血乳酸（Lac）；心脏超声指标——左室射血分数（LVEF）、左室舒张末内径（LVEDd）、左室收缩末内径(LVESd）、心输出量（CO）；生化指标——氧合指数（PaO2/FiO2）、B型脑钠肽（BNP）、C反应蛋白（CRP），及并进行对比分析。结果 患者于治疗后HR、MAP、CVP、Lac、LVEDd 、LVESd、BNP、CRP明显下降，LVEF、CO及氧合指数明显增加，结果均具有统计学意义(P＜0.05)；且血清中BNP水平与CRP的水平呈正相关（r=0.749, P < 0.05）。结论 连续性血液净化联合机械通气可以通过减轻容量负荷及清除炎症介质CRP来改善患者心脏功能，是抢救难治性心衰合并呼吸衰竭患者的有效措施。 |

| Achievement: Observe the clinical effects of continous blood purification combined with continous blood purification in the treatment of Refractory heart failure complicated with respiratory failure.
Method: Research the CBP combined with MV therapy for 42 patients of Refractory heart failure complicated with respiratory failure from Jan, 2012 to Dec, 2013. Record and compare the related data before and after therapy: heart rate, MAP, OI, Lac, BNP, CVP, LVEF, LVEDd, LVESd and CO.
Result: For all of 42 patients treated by CBP combined with MV therapy, MAP declined, heart rate declined, OI improved, BNP and CVP declined, LVEF and CO increased, LVEDd and LVESd declined, and clinical symptoms remitted, the results conformed to statistical evaluation.
Conclusion: CBP combined with MV therapy is effective for Refractory heart failure complicated with respiratory failure |
