          【】目的:研究BiPAP呼吸机对重症支气管哮喘患者动脉血气、生命体征、肺功能指标的影响,探讨其治疗效果。方法:以我院2014年1月至2016年12月间收治的100例重症支气管哮喘患者作为研究对象,回顾性分析其病历资料。对照组患者接受常规药物进行治疗(n = 48),观察组患者在上述治疗基础上行BiPAP呼吸机通气治疗(n = 52)。观察并比较两组患者的治疗效果。结果:(1)治疗前,两组患者动脉血气指标(PaO2、PaCO2、SaO2)、生命体征指标(HR、RR、MAP)、肺功能指标(FEV1、FEV1/FVC、PEF)、症状评分(反复喘息发作、咳嗽、胸闷、气促、呼吸困难)无明显差别(P>0.05),而在治疗后,观察组PaO2、SaO2、肺功能指标值明显高于对照组(P<0.05),而PaCO2、生命体征指标值、症状评分值明显低于对照组(P<0.05)。(2) 观察组患者治疗有效率明显高于对照组(92.31% VS 72.92%,P<0.05),且无严重不良反应发生。结论:BiPAP呼吸机应用于重症支气管哮喘的治疗,能有效促进酸碱平衡,改善肺功能及其症状,并恢复生命体征,安全有效。因此,该方法值得推荐应用。

          Objective: To study on the effect of BiPAP ventilatorSon the index about arterial blood gas, vital sign and lung function in the treatment process of severe bronchial asthma, so as to investigate the treatment effect. Methods: The 100 patients with severe bronchial asthma were treated in our hospital from January 2014 to December 2016. They were considered as study objects and the clinical data were analyzed retrospectively. The patients were treated with regular treatment method in the control group (n = 48), while the patients were treated with BiPAP ventilator on the basis of the control group (n = 52). The treatment effects were observed and compared. Results: (1) Before treatment, there were no significant difference between these two groups about the index of arterial blood gas (PaO2, PaCO2 and SaO2), vital sign (HR, RR and MAP) and lung function (FEV1, FEV1/FVC and PEF) and the symptom score (recurrent wheeze, cough, chest tightness, shortness of breath, dyspnea) (P > 0.05), but after treatment, the levels of indexes including PaO2, SaO2and lung function in the observation group was higher than that of the control group (P < 0.05), while the levels of indexes including PaCO2, vital signand the symptom score in the observation group was lower than that of the control group (P < 0.05). (2) The effective rate in the observation group was significantly higher than that of the control group (92.31%, VS, 72.92%, P<0.05), and there were no serious adverse reactions to occur. Conclusion: According to the application of BiPAP ventilator in the treatment of severe bronchial asthma, it can effectively promote acid-base balance and improve lung function and symptoms, with restored vital signs. It is safe and effective. Therefore, this method is worthy of being recommended.
